Tart Cherry Extract: What It Is and Why It Matters

Tart cherry (Prunus cerasus) is a botanical that has transitioned from traditional uses only to a subject of rigorous scientific investigation. It has emerged as a leading evidence-based nutraceutical. The diverse health benefits are primarily driven by a complex and synergistic array of polyphenolic compounds. The most notable of these are the anthocyanins. For example, there is a significant amount of cyanidin-3-glucosylrutinoside and cyanidin-3-rutinoside. These are highly bioactive. [1]

These bioactive molecules, along with others such as flavonols, hydroxycinnamic acids, and non-phenolic compounds like melatonin will exert potent multi-pathway (entourage) anti-inflammatory and antioxidant effects. I learned about the powerful ability of tart cherry extract to help with orthopedic conditions about 15 years ago or so. I started, and never stopped, heavy and deep research into nutrition and natural methods of improving health at that time. I found that modern medicine, especially my discipline of orthopedic surgery, was woefully inadequate in helping people.

Although we could make X-rays look better and fix broken bones pretty well, I found that many, if not most, patients never really felt better anyway. The ones that did always seemed to be healthier and to take better care of themselves. I do not recommend tart cherry juice, although I did in the past. I stopped recommending the juice due to the very high fructose load imparted by juice. The amount of fructose imbibed by one drinking tart cherry extract would directly counteract all of the benefits of the molecules found in the fruit. [6]

I personally take tart cherry extract to help my brain detoxify at night. I also take it to simply feel better. I know that it is a powerful antioxidant and anti-inflammatory.

The foundational problems that create chronic disease are oxidative stress damage to tissues and chronic, low-grade inflammation and its effects on tissues. I take tart cherry extract as part of my personal wellness routine so that I can fight these problems.

1. Muscle Recovery + Soreness Reduction

The compounds found in tart cherry extract have demonstrated an ability to decrease muscle damage and reduce pain. This obviously helps with recovery. Excessive levels of ROS after exercise can impair redox-sensitive calcium management, damage muscle membranes, reduce muscle contractile force and therefore impair performance.

By reducing the levels of ROS and rebalancing the redox reactions by way of the antioxidant powers, tart cherry extract molecules can improve your performance with exercise. This has been supported by studies in which blood markers of oxidative stress and inflammation were significantly reduced after exercise as compared to placebo.

In most studies that looked at the human exercise effects, supplementation with tart cherry reduces muscle soreness, strength loss, and markers such as DK and LDH after exercise. The benefits include better and faster recovery in strength training, marathons, and cyclinc. Meta-analyses demonstrate reduced inflammation as well. A 2023 study notes prolonged effects up to 12-hours after consuming the extracts.

There can be enhanced muscle recovery after engaging in strenuous activities. This extract may decrease muscle damage, reduce pain, minimize soreness, and improve recovery capacities after intense exercise. [2]
 

For exercise recovery and reduction of muscle soreness many believe that a loading phase of taking the extracts two to seven days prior to the strenuous activity helps. This is a commonly employed technique. For example, an extract consumed a week before a half-marathon, the day of, and 2 days after attenuated muscle catabolism (break-down) and reduced physiologic stress. Endurance performance benefits have been demonstrated with supplementation for seven days to a couple of hours prior to an event. [2]

Following a loading strategy, athletes have seen less pain and faster recovery of strength i the 24 to 96 hours after the exercise bout, as compared to athletes that did not supplement. Runners who drank the juice for seven days up to a race had three times less pain. However, I can’t recommend juice for the reasons we discussed previously. I strongly advocate for extracts. [2]

2. Cardiometabolic Health + Blood Pressure Support

Anthocyanins and other cherry compounds have demonstrated antidiabetic and antiobesity effects, increasing insulin production, ameliorating hyperglycemia, improving insulin sensitivity, and inhibiting some enzymes involved in carbohydrate absorption and glucose metabolism.

Taking extracts over a two day period was associated with a 35% reduction in recurrent gout attacks. More reductions were seen with three servings over the same time frame. It is suggested that if you are interested in assisting with sustained urate management, continuous supplementation, sometimes twice a day, is needed. A single dose may not prevent uric acid levels from rising again after about 24-48 hours. This makes sense as allopurinol itself is a daily medication and used for a lifetime. [5]

A quarter of people try cherry to help with gout. Cherries have antioxidant and anti-inflammatory properties along with the ability to downregulate the effects of NFkB and they are also hypourecemic. [5]

4. Neuroprotective + Cognitive Support

There are neuroprotective effects found with this extract. The phenolics and anthocyanins protect neuronal cells from oxidative stress and damage. They also exhibit anti-neuro-inflammatory properties. They stimulate pro-survival cascades, this potentially protects against cognitive aging and neurodegenerative conditions. [6]
 
This extract has also been studied for neuroprotection. Neuronal cells can be protected from oxidative stress and there can be lowered neuroinflammation with their consumption. Substances such as TNF-alpha and COX-2 are downregulated in microgial cells. Microglial cells are support cells in the brain and are abundant.

5. Antioxidant Activity and Pain Reduction (Anti-Inflammatory Powers)

The phytochemicals of tart cherry extract also produce powerful anti-inflammation for you. Anthocyanins can inhibit the cyclooxygenase enzymes (COX1, COX2). These are responsible for producing the prostaglandins that cause inflammation and pain. In addition to reducing the function of these painful enzymes, the extract will also suppress the pro-inflammatory signalling pathway of NF-kB. This means there is a downregulation of the pro-inflammatory cytokines such as IL-1beta, IL-6, IL-17, TNF-alpha, and prostaglandin E2 (PGE2).

The anthocyanins will also help to reduce the expression of vascular cell adhesion molecule-1, which is involved in white blood cell adhesion to tissues during inflammation. They will also downregulate the mitogen-activated protein kinase (MAPK) pathway. Even more, they downregulate the JNK1/phosphorylation pathway. This means that the overall signal noise inducing inflammation is turned down and you feel better.

Tart cherry extract will elevate plasma antioxidant capacity as measure by ORAC and FRAP. Markers of oxidative stress such as malondialdehyde and F2-isoprostanes were reduced in 80% of studies. Inflammation was decreased in 11 of 16 studies. Tart cherry extract will also lower levels of CRP, TNF-alpha, IL-6, and IL-8. A review study in 2025 confirmed these effects across chronic disease states such as arthritis and cardiovascular disease. The antioxidant capcity of these molecules are quite high, surpassing many other fruits (1145-2592micro mol TE/100g FW).

A dose of the extract can lead to increased plasma antioxidant capacity within one or two hours. This potentially lasts for up to 12 hours. The prolonged effect of a single dose is due to the upregulation of your own antioxidant enzymes but also due to the effects on the gut microbiome. The gut bacteria are capable of producing and reabsorbing some of the metabolites over time. [6]

While some anti-inflammatory markers are lowered within several hours of consumption, noticeable relief from joint pain and stiffness generally requires consistent, daily use. Clinical trials that have successfully shown such a reduction in inflammatory markers and symptoms in people with arthritis have used supplementation over periods of 3 to 6 weeks with some people noticing reduced inflammation and improved mobility in about 2-3 weeks of consistent use. 6. Improved Sleep

Four studies report that enhanced sleep quality and duration with tart cherry happens. Melatonin levels have increased and seem efficient. The benefits manifest in about three to seven days in these studies and many believe this is due to the low levels of melatonin in the extracts. There is about 2.1ng-13.5ng per gram. [8]

Improvement of sleep duration and efficiency have been reported after a week of supplementation in healthy individuals. Modest benefits were described in older and less healthy adults with insomnia after about two weeks of consumption. These are relatively lower dose studies and more work needs to be done in this field of research. I personally love to take tart cherry extract at night to help with my own sleep. That is ‘anecdata’ of course. A single, personal, and arguably biased example is never enough right? [8]

It is best to be consistent for a few eeks to properly assess the impact on your sleep patterns. The benefits are more about improving sleep architecture than in giving you a sense or feeling of ‘sleepiness’. Alcohol is well-known to give people a feeling of sleepiness, but it also damages sleep architecture. Therefore, feeling ‘sleepy’ should never be equated with true benefits. [8]

A study of older adults demonstrated nearly a full cycle of sleep (84 minutes) being added to a night with consistent use of supplements daily over two weeks. [8]

7. Oxidative Damage Control

Free radical scavenging is one way anthocyanins and phenolics work to benefit you when you consume tart cherry extract. These natural antioxidant molecules will donate electrons or hydrogen atoms from their hydroxyl groups, this helps terminate free radical chain reaction and prevents further cellular damage. The protection extends to DNA molecules, lipid membranes of cells, and proteins. The ability to provide antioxidant power is due to the chemical structure of the molecules. For example, delphinidin has a 3’,4’,5’-hydroxy group and this has high antioxidant capacity.

Anthocyanins are also good at chelating metal ions. This means they can sequester these molecules and render them less harmful and easier to excrete from your system. Metal ions are often involved in the production of free radicals and neutralizing them is known to benefit human health.

The Problems People Face: The Status Quo

Most of my patients complain about a few things:

 Most all of these patients will also have stiffness. The stiffness happens usually after longer time periods of being still. This happens when people first get out bed in the mornings. It also happens when they get out of a car after a long drive, or get up from their seat at the desk during working. [6]

My patients have goals that all of us have. They want to take care of their families. They have to carpool everyday. They have to work, get the kids to a million activities, and still make dinner. They have to clean up after dinner and help the kids with homework. Many of my patients take care of elderly, sick, or disabled relatives. The number of caregivers in our general population is astronomical and not talked about a lot. This means they are helping with moving and cleaning the dependent individuals. [6]

Most of my patients really want to do well at work and/or school. They don’t want brain fog. They don’t want the annoyance of being in pain. They want to feel strong. They want stamina. They want to be able to do it all and have it all. [6]

However, most of my patients do not sleep well. They do have brain fog. They are in pain. They are stiff. They do not eat a perfect, private-chef-derived diet. They are stressed to the max in short. [6]

Natural molecules that reduce inflammation do it in such a way that it is more comprehensive. That is, there is an entourage effect and various pathways are touched in the effort to provide anti-inflammatory and antioxidant benefits. For example, anthocyanins might reduce the activity of the cyclooxygenase enzymes, but might also increase activity of a gene transcription factor, such as nfKb and reduce inflammation that way. This allows them to achieve similar effects, but with diversification and this turns out to be safer based on existing reports. At the same time, this also means they take longer to work. [6]

Taking strong, synthetic, single molecule activity medications such as NSAIDs to mask pain or benzodiazepines to help with sleep, will give the illusion of the desired effect, but there are usually unintended consequences. GI irritation, kidney issues, and cardiac damage are well-known side-effects of NSAIDs as an example. [6]

Rather than simply masking symptoms with a very large change in a single pathway, and adding all of the risks of that method, natural molecules will allow a gentle reduction in pain and stiffness over time, as your body recovers and heals. [6]

NSAIDs vs. Tart Cherry Extract

NSAIDs have well-known and dangerous side effects such as GI bleeds, ulcerations, kidney damage, and cardiac damage. Single doses of NSAIDs have been shown to significantly damage the microbiome of the intestines as well. NSAIDs are also known to slow down healing of bone and connective tissue during the initial phases of healing.

For these reasons, I try to avoid such harsh chemicals if at all possible for my patients.

However, sometimes prescription medication is necessary. My hope for the patients that must take such medications is that perhaps more natural products can reduce the dose and frequency of these synthetics.

Tart cherry extract provides anthocyanin bioavailability for human health. It is always recommended that you consult with your treating physician, especially if you are on blood thinners, antibiotics, or if you have major chronic conditions that require medications.

Really, we always recommend that you discuss use of any diet or supplement with your treating physician. Again, high doses lack long-term data in human trials, though animal studies show no toxicity up to 9g/kg/day.

Why I Recommend Tart Cherry Extract to Patients

Pain + Osteoarthritis Support

Tart cherry extract significantly reduces self-reported pain symptoms and decreases inflammatory biomarkers, such as high-sensitivity CRP in patients with knee osteoarthritis. Some studies also indicate a reduction in markers of cartilage degeneration, like glycoprotein 39. [7]

The effects of tart cherry extract offer a potential way to help OA patients manage their disease with fewer adverse effects than traditional non-steroidal anti-inflammatory medications (NSAIDs). This is a significant and important consideration considering the NSAID side effect profile. [7]

Gout + Uric Acid Support

Tart cherry extract is strongly associated with a significant reduction in recurrent gout attacks. Gout is a painful condition due to deposition of excessive levels of uric acid crystals into joints and tissues. Tart cherry extract can lower serum urate levels and also increase urinary excretion of this molecule. This extract also potentially inhibits the enzyme xanthine oxidase. Xanthine oxidase is important in the formation of the uric acid crystals. Tart cherry extract i a valuable complementary or adjunctive treatment for those with this painful condition. [5]

When I tell my patients to ‘eat better’ and to incorporate the Mediterranean dietary pattern in their lives, I never expect a phone call the next day saying how wonderful they feel. Rather, this change takes time and the whole foods must replace all of the cellular structures that have been built by fast food, ultra-processed, and factory food over a lifetime.

Conversely, I do expect people dependent on narcotics to call if a prescription is not ready within hours of a clinic visit, because that is how synthetic molecules are designed to work. They alter a single molecule or a single receptor and make massive change, sometimes in addictive ways. Natural remedies and therapies will never, and should never, work the same way. [6]

Safety + Trust

As cherries are in the food supply they are not looked at in the same way as synthetic drugs. For food additives, if you are using anthocyanins as an antioxidant the ‘acceptable daily intake’ or ADI is about 2.5mg/day per the regulators. This would be about 175mg. That is for food additives and food chemistry. The European Food Safety Authority Scientific Panel has concluded that anthocyanins from fruit extracts are unlikely to be of safety concern. [10]

Animal studies have not identified a high dose problem. That is the no-observed-effect-levels (NOEL) was at 225mg per kg body weight for young rats. Animal studies have not identified any toxic effects of anthocyanins from currants, blueberries, and/or elderberries as an example. This is for amounts of up to 20mg/kg per day in rats, 25mg/kg per day in mice, 0.3grams/day for 15 or 90 days in guinea pigs and rats, 2.4% of body weight in beagle dogs, and 9grams per kg per day for 3 generations of rats, mice, and rabbits. What does this mean? The molecules in tart cherry extract seem very safe. [10]

There may be gastrointestinal issues such as nausea or diarrhea with use. Some people also have allergies of coursed. It should go without saying, but if you have an allergy to any part of tart cherry extract, you should not take it. [10]

Tart cherries are generally safe. There are no major adverse effects in trials up to 480mg or 16 ounces of daily juice over weeks. [10]

Clinical trials in humans consistently report good tolerability of tart cherry juice and concentrate. The majority of studies, including those up to three months, have found no significant deleterious effects. The most frequently mentioned adverse effect is mild GI discomfort. [10]

A single case report described a fully reversible decline in kidney function that was attributed to daily ingestion of very high doses of cherry juice concentrate. This implies that the extract is generally safe, but excessive consumption might pose a risk. However, this was a single case report. [10]

In a 28-day study of those with gout, only one adverse event was reported that was considered related to the extract itself. This was one instance of hyperglycemia, but the study used a concentrated juice. The American College of Rheumatology guidelines for gout mention there is a general difficulty in providing safe anti-inflammatory and anti-pain options for people with comorbidities like chronic kidney disease or diabetes due to potential interactions with the enzymes that metabolize drugs in the liver.
